INSTRUCTION FOR ONLINE HEALTH DECLARATION

(For visitors only)

In order to prevent the spread of Covid-19, all visitors upon entry Petrovietnam office are required to make health declaration. To do this, please follow these steps:

Step 1: Access the Health Declaration Management System at: http://khaibaoyte.pvn.vn or scan the QRcode at the reception.

Step 2: After accessing the online health declaration application, please fully update the information on the screen.

Step 3: Read carefully and fill correctly the information in the blanket in health declaration form (the box has sign “*” is required to fill).

Step 4: Click the “Submit” button, if the declaration is valid the screen will display your QR Code. Please save the QR code for check- in at the reception.

Step 5: When visiting Petrovietnam office, please use your QR code in step 4 for check- in at the reception.
